Cognitive ageing: what actually declines, what improves, and what you control

Ageing does not drain a single reservoir of intelligence. Different abilities follow different curves, and several of the strongest risk factors are modifiable.

Two curves, not one

Fluid abilities — reasoning about novel problems, processing speed, some aspects of working memory — peak in the twenties and decline gradually thereafter. Crystallised abilities — vocabulary, general knowledge, professional expertise — rise for decades and hold up well into later life.

Because a full-scale score mixes the two, the composite looks far more stable than either component. Any honest account of ageing has to talk about the profile rather than a single trajectory.

Why the data can mislead

Cross-sectional studies compare seventy-year-olds to twenty-year-olds today. Those groups differ in schooling, nutrition, health care, and test familiarity, so generational differences masquerade as ageing effects.

Longitudinal designs that follow the same people over decades — the Seattle Longitudinal Study is the classic — find later onset and gentler slopes than cross-sectional snapshots, though they have their own bias: the healthiest participants are the ones who stay in the study.

What changes in daily life

The most common experience is slower retrieval rather than lost knowledge: the name arrives twenty minutes late. Dividing attention becomes harder, and filtering out irrelevant information takes more effort, which makes noisy environments disproportionately tiring.

Prospective memory — remembering to do something later — is also sensitive, which is why external systems become more valuable with age. Using a calendar is an adaptation, not a symptom.

What is modifiable

Major reviews of dementia prevention identify a substantial share of risk attributable to factors people can act on: education early in life; hearing loss, hypertension, obesity, alcohol, smoking, depression, social isolation, physical inactivity, diabetes, air pollution, and head injury across mid and later life.

Hearing loss deserves particular attention because it is common, correctable, and easy to ignore for years. Straining to hear consumes cognitive resources and drives the social withdrawal that compounds decline.

Cognitive reserve

Reserve is the observation that people with more education, complex occupations, and rich social and mental engagement tolerate more brain pathology before showing symptoms. The brain has alternative routes to the same outcome.

The practical reading is not that puzzles inoculate you. It is that a life containing genuinely demanding, novel, socially engaged activity builds flexibility — and that this is worth protecting deliberately after retirement, when the structure that used to supply it disappears.

Testing older adults fairly

Timed tests penalise slower motor and perceptual speed, which can look like a reasoning deficit when it is not. Sensory impairment, unfamiliarity with computer interfaces, and fatigue all depress scores independently of ability.

If you are an older adult taking an online test, remove the obvious handicaps: use your glasses and hearing aids, choose a rested time of day, work in quiet, and read the untimed instructions carefully before starting.

When to be concerned

Normal ageing is gradual and does not derail daily function. Warning signs are change that is rapid, that others notice, that affects familiar tasks such as managing money or navigating a known route, or that involves confusion about time and place.

Those signs warrant a proper medical assessment, not a self-administered online test. Many causes — thyroid problems, B12 deficiency, medication interactions, depression, sleep apnoea — are treatable when found.
